Dynamic forms

Hello
I want to design a Website for laboratory. as we know there are in the laboratory more than 1000 test, as example : blood test , ceratin test, HIV test and etc. I need to design an environment that user can design a new test that not exist in database , this new test can be save to database and could change by user. User must could design limits for items in new test. as example : not entering number in textbox.
have you any idea or example or interface for my project??!
