deactivated certain element on form based on checkbox

Hi all thanx for reviewing my msg...and i'm thanking U in advance for your support and precious time... Basicly what I want to do is to deactivated certain form element (like textfield or listbox) based on checkbox condition...for exm I have 2 type of user registration in one form, Personal and Organization registration.On top of the form will be 2 checkbox...Personal and Organization registration.If the user were to select personal checkbox...the orgization checkbox and other element regarding orgaztion type of registration will be deactivated,,,can anybody help me with this issue
