Yes, I did. A few years ago I built a restaurant application. Complete with calculation (estimation) of ingredients of served items in order to keep track the inventory. However you could not ask for the source code since it was proprietary. Ownership of the codes is belong to my client back then.
But I could develop another one should you interested. Database could be MS SQL Server, or Interbase/Firebird, or MySQL, or MS Access, or SQLite (two of the latter only if you want the application to have local database, they are not really designed for multiple user access - security and accounting speaking).
