How to set up a Web-based Medical Web

I am working on a proposal to create a web-based solution for a local Medical authority.

The objectives of this project is to
1. Save times for doctors
2. Ease of filing applications and other materials through Web

Intro.
1. Doctors in private practices could log into the web and check for licences renewal, make payments, e-learning, and other services.

2. The Medical authority could then update these info and route these info to its database and legacy system.

3. This web is controlled by the Medical Authority.

How could I set up the whole system? What types of hardware and softwares do I need. Do I need to customize its existing system ?
