Does anyone have an example of a little contract that you have people sign before you take their picture to use for the web or other means?

I also need one for people under the age of 18.

I don't believe your looking for a contract, rather a release form. A
form that releases someone's likeness for reproduction.

It's for my church website (www.ibc-lex.org. I really want to put some pictures of some of our members on there, but there has been a big thing going around about churches putting up pictures of people without any form of contract or release form, especially with those under 18.

to come from a parent or guardian.

the way of 'Ned & Stacy' which I absolutely loved and it got

Surely they won't get rid of the simpsons! You know all the fanatics they'd have on thier hads(myself included). Everyone still loves that show.
