Looking for hosting with php,mysql,etc.

Hi everybody,
I am looking to host a website for my xbox live clan but cannot find a place to host my website.

I would like to have -
- 60mb webspace
- At least 800mb a month bandwidth
- php
- MySQL
- I have to be allowed to set up a forum.
- No Ads and Pop ups (I will link to your website if you wish)
- FTP access with a program, eg. WS FTP
I will also require subdomain:
eg. wingzero.host.com, URL

I will be setting up a phpBB2 forum and a phpclanwebsite template website.
