Comment / Ratings page

I need a comments page for a hair salon website where visitors can leave a comment and give a rating from 1 to 5 stars that will be displayed with the text comments.

There are a lot of free ones that have captcha and an admin to remove unwanted posts but couldn't find one that would add the stars.

So a form with fields for name, comment, drop down to choose stars, then an admin to remove posts.

Something like advanced guestbook http://www.proxy2.de/scripts.php which is free but with the star ratings added.
