Dynamic feedback form

Does anyone know if there is a script which can do the following?

Basically a feedback form spread over 7 pages (I presume using sessions). Page 1 would contain
* Empty field (for user to enter info)
* Drop down list 1
* Date (default date 5 days in advance of actual date)
* "Continue" button

User enters info, selects "Continue".
Page 2 contains:
* Drop down list 2 (information in list is dynamic, depends on users selection in Drop down list 1 on page 1)
* Drop down list 3 (information in list is dynamic, depends on users selection in Drop down list 1 on page 1)
* Empty field (for user to enter info)
* Drop down list 4
* Empty field (for user to enter info)

Page 3
* Yes / No radio button
* If user selects yes 2 more drop down lists are availble)

Page 4
* Blank text field

After the user finishes page 7, the details are emailed to me and user get a "thank you" email posted to them.

I am certain i did see this script somewhere but now i would like to use it i can't find it anywhere.

If anyone is willing to make the script for me then i could give them a banner ad on my website or a "powered by" on each page of the script for a couple months in return?

But, if you are looking to hire I am a freelance who could use a buck but I am not very cheap, might cost $50 when said and done, I saw a post in a forum here on devshed about people for $3 an hour if you want to be cheap (or need to be) check them out
