I am willing to pay

See if anyone could help me doing the following (I am willing to pay)

When a surfer came to visit my website, after entering his / her name, address, phone.... he clicks the submit button and his name and address will be entered in a preformated letter that will be emailed to say his specific state representative.

His information will be saved in another database and he would receive a thank note from me along with his letter.

Please inform me if you could help.

Need Help on a Petition Project

I am working on a petition project where by users enter their names and other pertinent information to sign a preformated letter supposedly to be sent to his/her state representative.

The program will look up the right representative that matched to the user's zip code (I already have an acess database with all the representatives' info), signed the letter, email the letter to the representative, saving users's information in another database, echoing the letter back to the user with a thank you note.

Please give me an estimate and advices too. This project is a for a non profit organization.

Thanks.
