PHP Programmer required

Hey all,

I'm a fellow php programmer who has too much work on at the moment to do a few simple tasks.

Basically what is required is a conversion of a pdf order form into a web form, which then formats the gathered information, and sends it via email to a specified address.

Clients requires it ASAP.

Here are the order forms that need conversion.

Please don't give me curry about the spaces in the file names ehe, their choice.

So you're client wants people to be able to fill those forms out on the web?

Would the scripts need to use any MySQL with the PHP?

Once one of those was done a sizable amount of the code could be copied and used again for the others.

I'm not very good at time estimates, but I think I could do it in two or three days.

Yes the client wants people to be able to fill out those forms on the web but there is no database integration at this point in time.

I'm able to do it by Thursday for $240 without the use of MySQL. The price includes a basic web-form layout, logo display, links to each form and a text-based email layout.

I'd also be glad to help if you still need it. I see no more than 5 hours of work per form regradless of how you want the data to reach the webmaster (mysql, email, text/CSV files). I can start right away and I usually charge $10 per hour.
