Urgent help making a speech form system

We have a form that is used by sales persons and we want to add a speech enabled way to complete the form by voice only. The system needs to have high accuracy so we think it needs to store voice profiles so the system can store the way the user speaks, like dragon naturally speaking.. my dev crew is not familiar with this at all, you can see the form btqnetlinux.com/form.zip
Please let me know if you can do this app, we have also found software other than what is offered by microsoft like http://www.chant.net/default.asp

If we need to buy that to make this happen we can.

it will need to send the output of the form to a network drive access db, nothing complicated there.
