You haven't provided us with any example log file entries to explain why you're upset. There may be a perfectly normal explanation for this; perhaps a spammer sent mail to a non-existant account on your server, and your server sent a bounce message to the spammer.

How did you reach this conclusion? What do you mean by "runaway"?

There's no such script. qmail-send and qmail-smtpd are two separate programs, invoked by two separate run scripts under supervise. Which one are you experiencing problems with? Without a copy of the script in question, no-one can help you solve any problems that might exist with it. Please provide an in-depth description of the problem that you are experiencing.

No, your ps output looks normal. There seem to be the correct number of supervise processes running (unless the "runaway supervisor process" is running something that doesn't have qmail in its name).
