Webmin and MySQL

Please help me with this problem.

I am trying to create a MySQL database by using the Webmin interface.
After clicking on the link to "create database" I am presented with a "New database options" table.
I then complete the "Database name" field, "Initial table" field, "Field names", "Data types", and "Type widths".
After doing all of this I click the create button and get the following message:

"Failed to create database : Missing or invalid database name."

Would someone tell me what they think that I am doing wrong?

Any advice would be greatly appreciated.

do you have pemision to create a database if you are on a virtual server you may be limited to a preassigned database and only alowed to make new tables within the database
