If you check the "Constrain Proportions" box, it should open at the right proportions. Typically when you open a file, PS will know the size of the file by reading the EXIF information and making a decision based on the pixel count. For example, a 3.0 megapixel picture is typically around 1984 x 1488 pixels (giving about 2.95 effective MegaPix). This can open in a range of sizes from 27.556" x 20.667" at 72 dpi to 6.613" x 4.96" at 300dpi.
