File Uploading/Logging

I'm looking for a small set of PHP3 routines that does the following:

1 - allows me to enter certain information (like a short text description) of a file

2 - uploads that file from my local hard drive to a remote ftp site

3 - stores the text description, filename, date and size in a MySQL db along with some keywords for later searching

Does anyone know where I can find a PHP3 script that does something similar to that? Or am I going to have to write one? :-)
