We want to develop a form in flash and use php. We are new to php and looking for some advice and direction.
All help is greatly appreciated!

The form we need to develop is designed to indicate vehicle preventive maintenance requirements. The form will have each item (from oil change to tire rotation) in a list format.
Next to each item will be a red, yellow or green check box.
We want the vehicle owner to be able to enter their license plate number (unique identifier) and their vehicles current mileage.
We will already have a populated file with previous mileage and service dates of said vehicle.
The form needs to look at the previous mileage versus current mileage and compare that to the recommended table of preventive maintenance service intervals. Then flag the item with either the red (past due), yellow (currently due) or green (okay) check box.

The form obviously needs to check mileage and date on some items, such as oil changes (ie: change oil every 3K or 3 months).

You'll have to do it in two pages. One with a FORM that gets the mileage and one with a TABLE to display the results.
