Creating Database Diagrams

When I first started my database I had a diagram that shows all the tables / elements and their relationships to one another. Of course as my project evolved I added / removed certain elements and didn't update my diagram along the way.
I was wondering if anyone knows of a program that will create a visual diagram of my database e.g. I specify the server my database resides on + l/p and it will generate a diagram, anyone know anything like that?

Rational Rose or Creately ?

At office we use Rational Rose to create database table from UML diagrams, Although I haven't done the reverse you might be able to do with it. Some of my friends mentioned that Creately UML Tools also have some sort of automation feature, so give them a try as well.

MS Visio is so far the best tool to do reverse engineer from any database and create ER diagram.
