Help with script

So can anyone maybe help me by making that script that I need??? I would be glad to give credit and a link to your website if someoe could. Plus I can learn by reading the script that goes exactly with what I need done.

I do know HTML but I have never done any kind of programing before. All this programming and stuff just confues me!
