In need of help

hello everybody,

does somebody have a script where the user can delete the records in a table from a PHP form.
The records should be deleted based on checkboxes.Like, when the records are displayed on the form, there would be a check box displaying at the end of each record and when the user checks this check box, and click submit, a value would be set to this row and in the next step, the user either can cancel the delete or really delete them.

If somebody had implemented this sort of application, pls send me.I would be really thankful to you.I've been trying this since couple of days and itz driving me nuts.Tomorrow is my last day for submission.Hope somebody would help me.
