Need someone to help with php email

Hi there all,

I can't afford to pay anyone but out of someone kindness of their heart could they please help me to link classes.php, class.phpmailer.php and class.smtp.php

I have all the code i just need help to alter it and make it so it can carry my email address..

I am very bad at PHP..

Files that need altering are attached.

When i try to use the form to email i just get this error message..

Once the user uses the webform her/she get a comfirmation which looks like this:

Feel free to contact for more details on site by emailing to this address .

Note: This is not a confirmation email. This is a notification email for your appointment registration on our site.

Also admin also get a copy of this information and the information is also blank.. WHAT CAN I DO???

But it is coming up blank and the database is not conecting...

I am using a linux server with FASTHOST.co.uk
