Need two web forms and a data base to go with them.

Hello,

I am looking for a programmer to make two web forms for me and a database (MySQL) to go with them.

Form 1 will be used for players to enter the results of their games. The data entered should be sent to their local game organizer. How it is sent and what format still needs to be determined.

Form 2 will be used by the game organizers to enter the data they get from their players. The data entered should go to the database which will output a every thing to a table for view on the web.

I have worked up how I want the forms to appear (would share them here, but can not on a new account) and can email or PM links to anyone interested in looking at the project in more detail. The forms are based off of Excel spread sheets that I am currently using, so I have all the math equations worked out (at least in Excel format).

I do have a small budget for this project, so lets talk about it.
