osCommerce Shopping Cart Setup Needed

I need someone who is VERY familiar with osCommerce to help me customize the functionality of it for a customer of mine. Here is a sample list of "other" functions that they need:

1. Customers must be able to provide me with their Credit Card security code during checkout! (this is found on the back of their card after the last 4 digits of the card number. This code consists of a 3 digits number);
2. Customers need to provide me with the name, or ID# of the person who introduced them to the products; they also may provide us with the name of the truck stop where the first found the products.
3. I need a section that allows customers to let me know if they want to sign up on autoship, so that we can have someone call them;
4. Customers with a P.O Box address must be able to select a USPS form of shipment, and they will need to know that they will be charged $2.05 handling fee;
5. When a customer orders a health kit, they will need to have a drop down menu that allows them to select one free bottle of either Mr. Trucker, or Lady Trucker, or Green Cream;
6. When customers order a health kit, they must be able to have a drop down menu that allows them to select the Turbo Charge Flavor of their choice;
7. We need to create a folder that allows booth owner operators to be rerouted to the wholesale prices;
8. We need to set up the following forms of payment: Credit Card, Bank Draft, and money Orders;
9. I need to be able to receive an order notification email at URL;

I know web design and graphics, not this PHP programming stuff! I'd like to form a long term relationship with a programmer to work on future jobs. I already have another cart, by the same company, needed but for a different site. Thanks!
