A good quiz script?

I have been looking for a good quiz script for a couple of weeks. I have found tonnes, but there either to hard for me to edit or not what I'm looking for. Does anyone know where I can find any type of script for the following?

There is a form (I can make this myself, as long I know what the fields are to be called to suit the script) and they select one answer from 3 choices. Then after filling in the whole quiz another page loads, with the results in, and an overall mark and percentage at the bottom. I found a javascript one which sort of did the job but there was no instructions about editing it and I didn't have a clue, lol.

Can any one think of any? or is able to write one?

The page ending that the quiz will be in will be .php, I dunno if this will have an affect on what can be run.

Have you tried hotpotatoes? it runs on your desktop and makes the quizzes into a number of different formats. You can upload them as complete pages or edit the layout in dreamweaver or whatever.
