Free form with simple admin backend?

hi

I'm looking for a simple form which allows users to submit a few details and a file... however rather than send this through an email to me, I would like it displayed and accessible by me in an admin backend.

Does anyone know of a script that does this... but which is easy to install and use? (oh and free )

I did find one that was free and did this exact same thing: http://www.perlservices.net/en/programs/allform/index.shtml .... although I tried installing it but just couldn't get it working.
