mysql form submission

hello everyone.

guys, am looking for a free php mysql script that can make online registrations possible
it would be great if u could help me find a script that can perform these tasks

- take inputs from a <form> & put them into a mysql database.
- also send the above data to a specified email address
- send a confirmation mail to the person who submits the forum.

hope am not asking much .. i basically need them for taking online registrations for an event.. it will really help if the gurus here could point me in the right direction
