Need a simple script for custom forms

Hey everyone,

Well let's get this part out of the way. I am not a web developer in anyway, and have very limited skills at html. My boss wants me to set up a page on our website with custom forms so that people can enter information and then the data will be emailed directly to him. I have found a few scripts that may do this however I have hit a few problems.

1) I'm stupid and cannot get most of the scripts to work
2) I do not know how to make new forms in the scripts
3) I am trying to find one where the page itself will send the information to my supervisor's email without the user having to open their email client.

So, if any of you feel like being generous and helping me out, I would really appreciate your skills and advice.

Why haven't you been able to get the other scripts to work? Did you follow the instructions on them? Make sure the script you try and install will work on your machine/server. A script written in PHP will probably be the easiest for you to setup as long as your server supports it. I would look here for scripts that might do what you want if you can use PHP:
