Upload image & change filename before storing

Hi Guys,

I'm looking for a PHP script that will allow a user to do the following:


1) Supply information with & upload a picture

2) Verify the picture is a jpg

3) Insert the information into a MySQL database

4) Change the the picture name to match the new database row id before storing the picture in a directory


Can anyone please help me out, I've been searching for a while now and I can't find what I'm looking for. I'm relatively new to PHP / MySQL I just can't seem to be able to put my own script together.

If someone could point me in the right direction I would really appreciate it.
