Figuring out the email will be upto you as I have no experence doing it. I can help you out along the way but I don't know anything about the specifics.
12 Questions could be a pain for you to update. Really it should be in a loop but it doesn't have to be. At this point I would say just try to get it to work, have it send out the email and all the formating is correct, then worry about getting the code to be a little nicer.
