adding the icon is easy.

1. create icon (16x16x256)
2. save as favicon.ico in the root folder of the site
3. add the following to the head:

<link rel="shortcut icon" href="http://www.mydomain.com/favicon.ico" />

bear in mind that this wil only work as long as the user has your icon cached. If they empty their cache your icon is gone and there ain't no bringing it back unless they rebookmark the page.

you'll need a third party prog make an ico file. I use LiquidIcon XP. It lets you do loads of stuff and best of all - its free!

Just create the icon in PS and then start LI, use the capture facility and click on your ps image - bingo

There are others which let you import different file formats. Michalengelo is a good all round icon and cursor maker but its shareware.
