Looking for a decent guestbook for my site

Hi -
I recently began designing my first website, and I'm looking for a somewhat chic/minimalist looking guestbook. All the guestbooks I found online, free or otherwise, seem pretty cheesy. Does anyone have any recommendations for a good looking guestbook with decent fonts - I don't know enough about design to make one myself, so I have no problem with paying for a guestbook with a cool design. I want the basic material for an entry - Name, city, state, country, etc - but I also want a field that has a drop down menu which would say favorite ----, with a host of options to pick from (movie, team, cd, etc.) before the general comment field. Any help would be greatly appreciated. Thank you!!

Do you have hosting that allows server side scripting, i.e. ASP / PHP etc?
If so, there are lots of guestbooks as www.hotscripts.com. Most will come with some sort of template (or HTML seperated from the scripting) so adapting the look and feel should be pretty straight-forward.
