I don't track minor revisions, but before I start on a structural/content change I change my editor settings so it makes a dated backup of the page (file.20050111.html, etc) in a special directory. I can revert back to a specific day's layout with a perl script that will replace the current file with the version matching that backup date (if any).
I have handwritten notes of general changes, but you could have a script to 'diff' the previous days backups and current files and log them.
