SWAT 4 Server

I am looking for a SWAT 4 game server for a customer of mine. He tells me that these servers can only be run on Windows or using VMWare, so keep that in mind. Here are some things necessary:

1) I need a company that specializes in game servers (it can be in other fields, but I don't want some company to run a game server on a hosting server).
2) In business for more than 3 months
3) Since it's a game server, 99.5% uptime + (I'll be generous)
4) Not using cogent lines, good ping
5) Money back guarantee, if possible (don't know if this is done for game servers)

He said he needs quotes for 12 and/or 16 players. Let me know your quotes, either post here or send a PM, and make sure your company fits the above requirements.
